Which passport form do I need?

Most adults can renew by mail with Form DS-82 if their most recent passport was issued when they were 16 or older, within the last 15 years, is undamaged, and is in their current name or they can document a name change. First-time applicants, children under 16, and anyone with a lost, stolen, or damaged passport use Form DS-11 and must apply in person at a passport acceptance facility. Expert Multi-Services helps you complete the correct form for $100, plus government fees paid to the U.S. Department of State.

What we do

  • Help you choose the right form (DS-11, DS-82, or DS-5504)
  • Complete the form accurately with you
  • Check your supporting documents and photo requirements
  • Explain where and how to submit, and how to pay fees

What we cannot do

  • Issue passports or guarantee processing times
  • Replace the in-person appearance required for DS-11
  • Collect government fees on the State Department’s behalf

Can I renew my passport by mail?

Usually yes, with Form DS-82, if your most recent passport was issued when you were 16 or older, within the last 15 years, is undamaged, and is in your current name or you can document a name change.

Who has to apply in person?

First-time applicants, children under 16, people whose passport was lost, stolen, or damaged, and people whose last passport was issued more than 15 years ago or before age 16 use Form DS-11 and must apply in person at an acceptance facility.

What do I need for my first passport?

You need Form DS-11, proof of U.S. citizenship such as a birth certificate or naturalization certificate, a valid photo ID with a photocopy, a passport photo, and the fees.

How do I update my passport after a name change?

If your passport was issued less than a year ago, you can usually use Form DS-5504. Otherwise, you may renew with Form DS-82 and a certified name change document such as a marriage certificate or court order.

I just became a U.S. citizen. How do I get a passport?

New citizens apply for a first passport with Form DS-11 in person, using the original naturalization certificate as proof of citizenship. We can help you complete the form.

Children under 16 apply in person with Form DS-11, and generally both parents or guardians must appear or give written consent. Bring the child’s birth certificate and the parents’ IDs.
